Official results of TOEFL Examination for students whose first language is not English
Name of office/ophthalmologist under which you will complete clinical hours OR application for our optional clinical rotations
Proof of health insurance and required immunizations if attending optional clinical rotations
Completed, signed, and initialed application
A non-refundable application fee of $50 due by May 31st for July start (Fall semester) or November 30 for January start (Spring semester). Late application fee after those dates is $100
Basics of Ophthalmology and Scribing Course Requirements
Prior medical or ophthalmological background is preferred but not required
